Instructions for Submitting Student Rating Forms for Processing

 
  Mid- and end-of-semester student rating forms are printed and processed by OIRA. As of July 1, 2007, this service will be provided free of charge to Syracuse University schools, colleges, and departments. Fees will continue to be assessed, however, for printing and processing of student rating forms related to sponsored programs and off-campus clients. These fees are described in Fees for Scoring and Analysis Services.

Preparation of Student Forms

Student forms for each course section should be kept together as a group. All forms must be face up with the heading at the top. Do not mix the groups together.

Header Sheet Preparation

Complete one FIESTA PINK Full-sized Scantron form (Header Sheet) for each group of forms as follows:

In the "Name" section

Write in and darken the corresponding bubble for:

In Columns:

Professor Name (designate University College courses by darkening "UC" at the end of the name) 1 - 18
Course Prefix 19 - 21

In the "Student Number" section

Write in and darken the corresponding bubble for:

In Columns:

Course Number A - C
Section Number G - I

In the "Additional Coded Data" section

Write in and darken the corresponding bubble for:

In Columns:

Total Course Enrollment Number (optional) A - C
ALWAYS darken the number ‘1’ J

Place the header sheet face up on top of its corresponding group of student forms. Please do not use rubber bands or paper clips to separate the groups.

If forms are not properly prepared or the header sheets are not completed correctly, the forms cannot be processed. You will be contacted to address the preparation concerns, which may lengthen the time needed for processing.

Reconciliation List

Submit a comprehensive list of the information on all of the Header Sheets. This list is used to compare the printed output with the coded information on the header sheets (see example below). The list must be organized as follows:

  • Alphabetically by professor
  • By course prefix for each professor
  • Numerically by course number for each course prefix
  • By section for each course number

If a professor has more than one course or section of a course, each must be listed separately. University College courses must be listed separately from main campus courses. The total number of entries on the list should equal the total number of groups of forms being submitted for processing.

Proof the list carefully as it is considered the default should any discrepancies arise. If a reconciliation list is not submitted, any possible coding errors on the Header Sheets will not be found and corrected.

Deliver all groups of forms with header sheets on top and the Reconciliation List to OIRA at 400 Ostrom Avenue. Parking for drop-off and pick-up is available.
